Our Home Health Aide Evaluation is a comprehensive assessment designed to determine the level and type of non-medical support a client may need to safely and comfortably remain in their home. This evaluation helps us develop a personalized care plan that ensures dignity, independence, and quality of life for each individual.
During the evaluation, we assess the following key areas:
Personal Care Needs – Bathing, grooming, dressing, toileting, oral hygiene, and mobility assistance
Medication Support – Ability to manage and adhere to prescribed medications, including reminders and monitoring
Nutrition & Meal Assistance – Dietary needs, meal preparation, feeding support, and hydration habits
Emotional & Social Well-being – Level of companionship desired, mental stimulation, and emotional support
Household Management – Assistance needed with light housekeeping, laundry, and home organization
Errands & Daily Living Support – Ability to manage errands and household supplies independently
Health Monitoring – Need for vital sign observation and support with rehabilitation exercises
Home Safety & Fall Risk – Environmental safety, mobility limitations, and risk of falls
Scheduling & Organization – Support needed with coordinating appointments, managing paperwork, and daily structure
